Advance Care Planning is a way to:
- Make sure you receive the level of care you request – no more and no less
- Let others know what to do when you are not able to express your wishes
- Authorize people you trust to make medical decisions on your behalf
What Is An Advance Directive?
An advance directive is a legal document that:
- Names your healthcare agent (a person whom you trust, respect and can be your advocate to make medical decisions on your behalf, if you are unable to do so yourself)
- Identifies your healthcare preferences
- Provides guidance for your healthcare agent and healthcare providers
Why Is Advance Care Planning Important?
It is a way to make sure your wishes are known and followed. Without it, your family or loved one(s) may not be able to make decisions on your behalf and could be forced to involve the court system to make these difficult decisions. That can involve legal expenses and a potential delay in your treatment.
Who can complete Advance Directives?
Any individual 18 years or older can create an Advance Directive.

Advance Care Planning is a FREE service that is offered through Sauk Prairie Healthcare.
How Do I Complete an Advance Directive?
- Schedule an appointment with our Care Continuum Team at Sauk Prairie Healthcare, Clinic or Hospital Campus by calling 608-643-7589
- Prior to your scheduled meeting, review the Advance Directive packet.
- Watch our informational video for guidance on completion of documents here
- Come to the appointment prepared with the contact information for your elected healthcare agents and any questions you may have.
