MyHealth portal for visually impaired
MyHealth is the easy and secure way to manage your healthcare online, anytime. You can:
- Access your health record online, including lab test results and immunizations.
- Securely message your primary care provider.
- Print your medication list.
- View upcoming appointments.
- Review your past visits and health history.
MyHealth also lets you view your child’s or another adult’s health information (with their permission) and you can give a spouse, parent or other adult access to your account. Child age restrictions may apply.

First-time user? Enrollment is easy:
Activate your account now, if you:
1. Know the last four digits of your Social Security number and it is on
file with Sauk Prairie Healthcare OR know your medical record number
AND
2. Have already given your email to Sauk Prairie Healthcare's registration staff
