MyHealth Patient Portal is provided as a secure, online tool to help you manage your healthcare and is not intended to replace the services provided by your professional care team.
Under no circumstances shall MyHealth Patient Portal be used to convey medical advice or treatment. Patients must contact their provider's office for these patient services.
The electronic patient health information (ePHI) communicated via MyHealth Patient Portal is confidential and protected under the Health Information Portability and Accountability Act (HIPAA).
You may request to grant a trusted family member access to your health records via MyHealth Patient Portal. This "proxy" request must be made in writing and verified with proof of identification. (Children will be automatically removed from their parent(s) account(s) at the age of 12 and can be added back on in most circumstances with written authorization of the child.)
You may choose to print portions of your health record from MyHealth Patient Portal. This information then becomes yours to protect and hold confidential.
You may choose to download your health summary to your personal computer or handheld device. The file then becomes your property to secure and keep confidential.
You may choose to transmit a copy of your health summary to a health care provider using the eHealth Direct functionality in MyHealth Patient Portal. In the process, your health records are encrypted and any breach of security is reportable and punishable under penalty of law.
By accepting these terms and conditions, you are agreeing to keep your password confidential. You may change your password at any time using the Change Password button on the Profile screen in MyHealth Patient Portal.
