Sauk Prairie Healthcare's Diabetes Self-Management Education program, certified by the American Diabetes Association, is for individuals of all ages as well as their families. Our goal is to empower you with the knowledge and skills you need to make informed, self-directed behavior changes to manage your diabetes. Your success will depend on attaining your goals, maintaining good health, and reducing your risk of life-altering complications.

To participate in our Diabetes Self-Management Education program, ask your physician for a referral or schedule an appointment with our nurse practitioner with specialty in Diabetes Management.
